Process Hazard Analysis (PHA);

Description: : PHA includes all activities involved in hazard identification and risk assessment throughout the lifecycle to protect employees, the public, or the environment consistently within the organization’s risk tolerance. PHA addressing the following three (3) questions:

  1. What can go wrong? ➔ Hazard Scenario
  2. How bad could it be? ➔ Hazard Severity [S]
  3. How often could it happen? ➔ Likelihood [L]
    Risk [R] = Hazard Severity [S] x Likelihood [L]

Layer of Protection Analysis (LOPA);

Description: LOPA is a semi-quantitative risk assessment used to determine if there are enough protection layers against an identified hazard scenario. LOPA is also used to allocate a Safety Integrity Level (SIL) to Safety Instrumented Functions (SIF) within the Safety Instrumented System (SIS) as a protection layer.

Human Factors Engineering (HFE);

Description: HFE incorporates human factors to the design, operation, and maintenance of processes, systems, and tasks, for safe and effective human use. It also ensures opportunities for human error have been assessed and adequately addressed.

Pre-Startup Safety Review (PSSR);

Description: PSSR verifies that new processes or prolonged out of service processes are safe prior to startup.

Description: SIS is an Independent Protection Layer (IPL) designed to instrumentally prevent a hazard and take the process to a safe state based on predetermined parameter conditions. SIS is complementary to the Basic Process Control System (BPCS).
